Chronic Conditions

We work to prepare and empower patients and their families to make informed decisions about their health. We are committed to providing tools, resources, and support to improve a patient’s ability to self-manage their condition and live a healthier lifestyle.

Healthy Choices

Through family education and community advocacy, Healthy Choices strives to improve the nutrition and physical activity environment for adults and children in Milwaukee’s south side, so they can enjoy a healthier life.


The HIV Department is committed to the delivery of high quality culturally sensitive care in Milwaukee and surrounding areas, based on the National Model: Prevent-Test Link-Treat and retaining our patients in care to improve the quality of life of people living with HIV/AIDS and prevent new infections.

Parenting Resource Center

The Parenting Resource Center offers a variety of services such as care for children during appointments, car seat classes and parenting classes, on-site for patients and their families.

Social Services

The Social Services Department works across the organization to bring extra care, advocacy, and resources around housing, safety, family and social relations and basic needs to our patients and community. 

WIC | Women, Infants and Children Nutrition Program

WIC provides nutrition education, breastfeeding support, supplemental nutritious foods and referrals to other health and nutrition services.

Health Equity Summit:
Empower a Healthier Milwaukee



We’ve made the difficult decision to cancel our rescheduled 2020 Health Equity Summit; Empower a Healthier Milwaukee due to COVID-19 related precautions and restrictions around large gatherings. We appreciate your interest and are disappointed that it will not take place as planned.
We will be holding a Health Equity Summit in the spring of 2021 that will include the previously planned conversation on empowerment along with other dynamic topics under the broader umbrella of health equity innovation. Although we plan to hold the 2021 Summit in person, we acknowledge the significant uncertainties we face and are prepared to adapt quickly if needed.
