Sixteenth Street clinic opens on Milwaukee’s south side & is the first community health center in the state

Flagship Chavez clinic expands to double in size

1984

Sixteenth Street is designated as a Federally Qualified Health Center (FQHC) — a community-based non-profit health center/clinic that provides primary care for medically underserved areas and populations.

 

Patients receive care regardless of their insurance or immigration status or the ability to pay.

1994

The first-ever “Celebrity Roast” fundraising event is held

 

The Department of Environmental Health is established. Still in existence today, the Department focuses on improving the natural and built environment where our community lives, learns, works, and plays.

2010

First Joint Commission accreditation received (subsequent accreditations in 2012, 2015, 2018)

2015

First in-school medical clinic established at St. Anthony schools

2018

Health Equity Summit launches

 

Layton clinic opens (includes medical & behavioral health services) and the Child & Adolescent Day Treatment program is established (the only bilingual program of its kind in Wisconsin)

2021

National Ave. clinic opens — it’s the only Sixteenth Street clinic solely dedicated to behavioral health.

 

Today, we have more than 60 behavioral health providers caring for our patients.
