CCS (Comprehensive Community Services)
At Sixteenth Street Community Health Centers, we believe recovery is possible – and that it looks different for everyone. Our Comprehensive Community Services (CCS) program is a voluntary, recovery-oriented program designed to support adults living with mental health and/or substance use challenges.
CCS meets you where you are and helps you build the skills, stability, and support needed to move forward on your own terms.

What is CCS?
Comprehensive Community Services (CCS) is a Medicaid-funded program that provides individualized, community-based support for adults on their recovery journey.
This program is centered around your goals and is not a one-size-fits-all plan. Together, we work to strengthen the areas of life most impacted by mental health or substance use needs.
Who is Eligible?
To participate in CCS, individuals must:
- Be 18 years or older
- Live in Milwaukee County
- Have Medicaid coverage
- Have a diagnosed mental health and/or substance use disorder
- Be motivated and interested in their own recovery journey
CCS is voluntary and works best for individuals who are ready to actively participate in building their path forward.
What Does CCS Offer?
CCS provides support across multiple areas of life that may be affected by mental health or substance use challenges, including:
- Housing stability
- Employment support
- Skill development
- Health and wellness
Services are flexible and can take place in our clinic, in your home, in the community, or virtually.
Our goal is to remove barriers and ensure services are accessible and practical for your daily life.
How is CCS Different?
CCS is not traditional case management. Instead, it is a collaborative, team-based approach where you choose your goals and help shape your treatment team. Recovery is not a quick fix. It takes time, effort, and support. Through CCS, you will learn the tools and skills needed to build independence and long-term stability. We walk alongside you while you lead the way.
What Are the Next Steps?
If you are referred to CCS, an Intake Coordinator will contact you by phone to schedule your intake appointment.
It is very important to answer this call, as the intake must be completed within 30 days of referral. The intake process can take up to four weeks to complete.
