APP Fellowship
Sixteenth Street is proud to offer a year-long Advanced Practice Provider (APP) Fellowship designed for early-career Nurse Practitioners (NPs) and Physician Associates (PA-Cs) who are passionate about delivering high-quality, culturally responsive care in an FQHC setting.
Our fellowship supports APPs who embody our mission and values while building the clinical confidence, professional identity, and leadership skills needed to thrive in community-based primary care.
The program welcomed its first two fellows in October 2025 and will continue to accept two to four APPs annually.

Our vision is to create a nationally recognized, community-based educational program that facilitates and strengthens the breadth of practice and abilities of APPs in an ever-evolving healthcare system serving under-resourced and diverse populations.

Program Structure & Leadership
The APP Fellowship is a 52-week, full-time program built around progressive autonomy, team-based care, and intentional mentorship.

Each fellow is paired with dedicated coaches and clinical mentors who provide consistent guidance throughout the year.
Developing Clinical Excellence
Fellows develop their own patient panels while working within a highly supportive clinical environment.
- First three months include 1:1 supervision by an APP or MD
- Dedicated availability for:
- Real-time clinical questions
- Review of physical exam findings
- Charting and documentation feedback
- Gradual progression toward independent practice while maintaining team-based support
Didactic Learning
Protected time is built into the schedule for both synchronous and asynchronous learning, including:
- Clinical reasoning exercises
- Medical literature review
- Focus on common, high-risk, and self-identified learning needs
- A leadership and career focused book club
Clinical Rotations
Fellows rotate through key service areas to strengthen interdisciplinary collaboration and population health skills:
- Diabetic Nurse Educator
- Asthma Education
- Case Management
- Social Services
- Medication Assisted Treatment (MAT)
- HIV Services
- WIC
- Women’s Health
- Weight loss clinic
Continuity Clinic & Team-Based Care
Fellows will receive increases in patient volume, along with regular feedback and performance check-ins. They will follow a structured, phased clinical schedule that ensures confidence is gained while delivering safe, high-quality care. There will be six half-day clinics per week.

Graduation Requirements
To successfully complete the fellowship, fellows must:
- Complete the full 52 weeks of the program
- Complete a minimum of 1,350 patient visits (tracked quarterly)
- Transition to and maintain a schedule of ten patients per session for eight weeks prior to graduation
- Meet expectations for documentation, in-basket management, and professional responsibilities
