Sixteenth Street is proud to offer a community pharmacy at its S. 16th St. location (1032 S. 16th St., Milwaukee, WI 53204).

Our Sixteenth Street Community Pharmacy is open 8:30 a.m. — 6:00 p.m. Monday through Friday. We are proud to employ bilingual pharmacists and pharmacy technicians who can serve you in English or Spanish. Our pharmacy serves as a one-stop shop for our patients, making it easier than ever for you to pick up your medications following your visit.

Each pharmacy patient will have their medication list reviewed for cost-saving opportunities, which allows us to ensure you are paying the lowest out-of-pocket cost for your medications.

The pharmacy is in-network with all major pharmacy insurance plans aside from Humana. We also proudly support our Sixteenth Street Sliding Fee Program for uninsured and underinsured patients.

The pharmacy offers the following services for patients:

  • A free medication delivery program for eligible patients. Please contact our friendly Sixteenth Street pharmacy staff at (414) 999-4000 to confirm if you are eligible for this free service
  • Participation in our personalized Medication Adherence Program for patients struggling to reach their health goals with their medications
  • Ability for patients to utilize the MyChart app for Apple and Android to view their medication profile, request refills, and communicate with the pharmacy

The pharmacy operates on EPIC Willow software which communicates with your Electronic Medical Record. This allows us to help you by:

  • Improving our pharmacy team’s ability to ensure your medications are as safe and effective as possible
  • Allowing your provider to view medication adherence data to improve clinical decision making
  • Allowing us to resolve medication refill issues and prescriber questions quicker, which ensures you have your medications available when you need them
