Social Services Specialists can help determine if a client may be eligible and will assist with enrollment into Badger Care Plus including coverage for children, families, childless adults, and express enrollment for qualifying pregnant and child cases. Elder, blind and disabled Medicaid is handled by Social Security. We also have a Certified Application Counselor to enroll in Marketplace for existing Milwaukee patients only. TB Medicaid enrollment services are also available for established active medical patients.
Sixteenth Street submits applications to the state, and the state makes determinations on the case. In order to maintain coverage, clients will need to submit any documentation required by the state and comply with any review requirements or instructions by the state.
Patients seeking assistance with housing, emergency shelter, food, clothing, furniture, forms for disability applications (SSI), food stamps, payee issues, medical bills, vital records, paternity, child support, legal, educational resources, employment, and AODA will be directed to other community resources via 211 Impact, resource directory lists, etc.
