COVID-19 Vaccine Information for Non-Patients

Groups eligible for the vaccine per state guidelines include:

  • Adults who are 65 years or older
  • Adults who are 18 year or older with certain health conditions
  • Adults who are 16 or older who live in certain zip codes (53204, 53205, 53206, 53209, 53215, 53216, 53218, 53223, 53224, 53233)
  • Education and child care workers
  • Individuals enrolled in Medicaid long-term care programs
  • Some public-facing essential workers
  • Non-frontline essential health care personnel
  • Facility staff and residents in congregated living settings

What to Expect at Your Appointment

  • The vaccination site is located at Kosciuszko Community Center, 2201 S. 7th St., Milwaukee
  • The site has accessible parking and is surrounded by several nearby bus stops.
  • Please arrive no more than 15 minutes ahead of your scheduled appointment
  • You must arrive to your appointment with your confirmation number. This will be provided to you by email once you complete the registration and consent form
  • Vaccines are free of charge
