Our Medical Records office is open Monday – Friday, 8:00 a.m. – 4:30 p.m. A valid form of ID is required to receive medical records. Valid forms of ID include:

Passport
Driver’s License
Birth Certificiate
State-Issued ID

USB/Flash Drive: $6.00 + 10% of total costs for copies required in seven days or less

Notarizing: $5.00

Vaccines: $2.00
