Sixteenth Street provides an array of health services for men and women, families and children. As a Community Health Center, these services are provided in a neighborhood setting, with respect for the language and culture of the residents of the community and are made available without regard to insurance status.
Family Health Care
Adult medical services to treat illness and promote health for men and women of all ages. Special case management and patient education focusing on patient self management of diabetes, asthma and HIV.
Pediatric care for infants and children including well child checks, immunizations and health screening for lead poisoning as well as oral health promotion.
Women’s health services including the full range of reproductive health services including contraception, natural family planning, pre-natal care and delivery services. Seven certified nurse midwives provide these services along with Sixteenth Street’s family practice medical staff.
Dental services are available to Sixteenth Street’s uninsured patients through a grant funded referral program with Dental Associates. In addition, Marquette University’s School of Dentistry operates an oral health clinic at Sixteenth Street’s Parkway location at 2906 S. 20th Street.
The complete range of out-patient mental health services for all ages provided by 17 staff members (13 bi-lingual in Spanish and English) including psychiatrists, psychologists, family therapists and social workers.
Specialty care providers see Sixteenth Street patients at regularly scheduled clinic sessions. These practice areas include ophthalmology, obstetrical-gynecology, HIV, petdiatric gastroenterology and podiatry. On staff expertise includes specialist training for liver disease and hepatitis C treatment.
HIV Outreach, Prevention and Treatment Sixteenth Street is a Ryan White Title 2 & 3 HIV provider. Outreach staff conduct educational training in schools as well as counseling and testing for high risk populations. Case management, client support services and a needle exchange program are also features of this program.
Physical therapy services are available in partnership with Extendicare Health Services to care for patients with repetitive motion or work related injuries as well as assistance with rehabilitation programs.
Sixteenth Street Community Health Center accepts all HMO and private insurance patients, including Title 19 and Medicare.
Patients without insurance will be charged based on family size and income. Proof of income is required. A financial counselor is available on site to assist you. Patients are encouraged to bring insurance information with them to ensure accurate billing.
