Quality of Care Indicators

 As a Community Health Center, Sixteenth Street reports health care quality measures annually to the federal Department of Health and Human Services, Bureau of Primary Health Care.

These measures include:

Trimester of Entry into Prenatal Care
     In 2012 76% of patients entered pre-natal care in the first trimester

Childhood Immunizations
     89% of children under age two were up-to-date with immunizations

Cervical Cancer Screening
     79% of women were screened for cervical cancer

Hypertension under Control
     77% of patients had their hypertension under control

Diabetes under Control
     70% of patients with diabetes had A1c levels under 9

Low Birth Weight
     8% of babies delivered by Sixteenth Street providers were below normal birthweight
