Poverty impacts people’s health tremendously. As the only source of healthcare for much of Milwaukee’s most vulnerable population, the mission of Sixteenth Street Community Health Center lies in addressing the roots of poverty beyond the exam room to improve the health of our patients any way we can: medical and behavioral health care, involvement in the community, and collaboration and coordination with partners.
Our work is guided by the belief that:
- Everyone has the right to high quality, accessible healthcare that is free from linguistic, cultural and economic barriers.
- Care must be delivered beyond our clinic walls in order to give individuals and families the support, services and self-empowerment they need to make wellness a sustainable reality.
- Building healthy communities means more than health care; it means narrowing health disparities and access gaps and improving the environment where people live, learn, work and play.
- A focus on prevention, rather than a treatment model of care, is the foundation of improving population health.
