Quality of Care Indicators

As a Community Health Center, Sixteenth Street reports health care quality measures annually to the federal Department of Health and Human Services, Bureau of Primary Health Care.  As an agency we are accredited and certified by various bodies that distinguish our quality improvement measures, state-of-the-art performance standards and provision of patient-centered, coordinated care.
