Sixteenth Street is accredited by The Joint Commission, which means that our patient care and facilities meet national standards for quality and safety. Our accreditation was renewed on June 14, 2012 and is valid for up to 39 months following that date.
On April 30, 2013 Sixteenth Street received word from the National Committee for Quality Assurance (NCQA) that Recognition as a Patient Centered Medical Home (PCMH) Program Level 3 has been granted to our organization
And, just in time for “Bike to Work Week” May 13-17, 2013 Sixteenth Street Community Health Centers have been awarded Bronze level recognition as a Bike Friendly Business from the League of American Bicyclists.
Sixteenth Street is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
