About Sixteenth Street Community Health Center
Case managers, outreach and education specialists and eligibility assistance workers helped 4,284 people at 17,833 individual appointments deal with complicated medication plans, learn how to manage their chronic illness or enroll in programs or services for additional assistance.
Sixty-three percent of Sixteenth Street patients reported incomes below the federal poverty level and 30% were without health insurance. 80% of patients are Hispanic or Latino, 15% are White, 2% are African American, 2% are Asian and 1% unreported or other. 63% of patients were best served in a language other than English.
1,601 women received pre-natal care at Sixteenth Street and 919 deliveries were performed by Sixteenth Street providers. 69% of these deliveries were done by the Certified Nurse Midwives in Sixteenth Street’s women’s health department. 94% of these babies were at or above normal birth weight.
