Welcome to Sixteenth Street Community Health Centers
With five service locations, and employing nearly 400 people, Sixteenth Street Community Health Centers have provided high quality health care, health education and social services to low-income and culturally diverse residents of southeastern Wisconsin since 1969. Comprehensive health services include adult and pediatric medicine, behavioral health services, women’s health, HIV prevention and treatment and physical and occupational therapy. The Waukesha Community Health Center (wholly owned and operated by Sixteenth Street) opened to patients in 2012. In response to the needs of the community, the Waukesha Community Health Center now provides the full range of out-patient mental health services in addition to primary medical care for all ages.
