Welcome to Sixteenth Street Community Health Centers

With five service locations, and employing nearly 400 people, Sixteenth Street Community Health Centers have provided high quality health care, health education and social services to low-income and culturally diverse residents of southeastern Wisconsin since 1969.   Comprehensive health services include adult and pediatric medicine, behavioral health services, women’s health, HIV prevention and treatment and physical and occupational therapy.  Meet our bi-lingual medical and behavioral health providers.

Our Waukesha Community Health Center opened to patients in 2012.  In response to the needs of the community, the Waukesha Community Health Center now provides the full range of out-patient mental health services in addition to primary medical care for all ages.  New patients are welcome! How our programs make a difference

  • Midwifery 500

    Last year Sixteenth Street’s Certified Nurse Midwives and Family Practitioners delivered over 800 babies. More than 95% of these babies were at or above normal birth weight. Our midwife practice was cited as having the Lowest C-Section Rate among large practices by the American College of Nurse Midwives Benchmarking Project.

    Outreach workers make visits to older homes in our community to help families remove or abate hazards from heavily leaded paint that can poison their children. Since 1994 when the program was started, the lead poisoning prevalence rate for children served by the Lead Poisoning Prevention Program has dropped from over 40% to just over 2%.

  • BikeDay 500

    Pediatrician David Waters rides his bike to work, as do at least eight other members of our staff. Planning is underway for the 4th Annual Southside Bike Day. In 2014 600 neighborhood residents turned out to get their bikes tuned up, get fitted for a helmet and grab some exercise in the park.

  • HealthyChoice1

    Healthy Choices, or Elecciones Saludables, is a 12 week program for families to help them fight obesity with a Learning component (nutrition, physical activity and stress management); a Relating component (participants connect and communicate their fears, needs and challenges related to health); and a Doing component (participants commit to making eating and lifestyle decisions. Nearly 68% of adults and 29% of children lost weight through their participation in the program.
